MSHSAA has passed a by-law that allows for a sports physical to be good for 2 years (730 days). 

Any student who had a physical completed for the 2022-2023 can use that same physical for the 2023-2024 calendar year.

Any athlete who is going to be getting a new physical for the 2023-2024 sports year must use BOTH of the following forms:

MSHSAA Medical History Form

MSHSAA Pre-Participation 2-Year Physical Form
